Advance Health Care Directive
As you make the final decisions about your surgical care, it is important to have all of your questions answered so you can make your own healthcare decision. If you are unable to speak, you can arrange ahead of time to have someone you have chosen to be your personal agent on your behalf to make your healthcare decisions, such as a healthcare surrogate or closest available relative. These directives are to ensure patients have control over their medical care for as long as possible.

Medical Records
If you require a copy of your medical records, please call our office for instructions. If you require medical records from the hospital, please call the Medical Records Department, and they will give you instructions.
Short- and Long-term Disability, Family Medical Leave, and Work Release paperwork can be prepared for you may have a small, nominal charge – please ask us.
